If you are new to counseling or have been in and out of counseling your whole life, during your first session your therapist will go over a from call “Informed Consent.” This form will be a review of the policies, for that individual therapist. Normally the informed consent will be talked about what to expect along with other important aspects of the counseling process. Informed Consent
Welcome to Richer Life Counseling. This informed consent contract holds important information about my services and business policies and how they may affect you. At any time during our sessions, we can discussion any questions that you might have. Once you sign this document, it will become a binding agreement between us and also provide your consent for us to begin therapy.
What to Expect
Therapy is the development of individual skills and brings a unique insight into interpersonal thoughts and feelings. Therapy can result in a benefit to you and can challenge your beliefs and attitudes and could create, maintain and worsen feelings of depression, anxiety, panic, anger, frustration. Also, therapy has the potential to help gain new or deeper understanding about your issues and learning new ways to cope and solve them
I use a client-centered approach to therapy and if you want to know more about my theoretical perspective on change and how therapy will work, please bring up any questions during the session.
Meeting
Session last 50 minutes and will begin at a predetermined agreed upon time.
Fees and Payments
Session fees are a predetermined amount decided when booking your first session. Payment in full need to be made at the beginning of each session unless previous arrangements have been made. Payments can be made in cash or charged to your credit card.
Cancellation policy
24-hour advance notice is needed on any cancellation. The only way to cancel a session is by a phone call or text to (702) 518-1546. A $25 cancellation fee will be applied to a credit card that is on fine if less than a 24hr notice is given. Full session change will be applied to credit card on file for any no show sessions.
Email Usage
Email correspondence is NOT confidential. Though internet security measure can be effective, it is never 100% seal proof.
• Email is not secure.
• Anything stated in an email from you will be disused in session and in session only.
Audio/Video Recording
As a student-Intern audio/video recordings are routinely made (and kept confidential) as part of the treatment and supervision process. Audio and video recordings of counseling interviews maybe also be used for educational purposes, with the identity of the client concealed.
Contacting Me
Clients can contact me at (702) 518-1546 throughout the week from 8 am to 8 pm. I will do my best to answer within a 24 hour period. During weekend or holidays, I will not return calls, until the day after a holiday or the next business day.
In the case of absence, I will give you reasonable notice if possible if I am out of the office for an extended period of time. If needed refers can be made available.
Emergencies
If you are experiencing a life-threatening emergency and need to talk to someone immediately you can call 911. Or the Suicide Prevention Hotline at (800) 273-TALK (8255), the police, or your local hospital emergency room and ask for the psychologist or psychiatrist on call.
Confidentiality
Everything you say and share in session is strictly confidential. However, there are some exceptions to the rule of confidentiality.
By law, I am required to report
•Threats of harm to others
•Threats of harm to oneself
•Suspected child or elder abuse (past or present) – if you are a victim or perpetrator.
•By court order
Other exceptions include:
• Per you signed release
• I may discuss your case with a supervisor or peer counselors in order to provide excellence in the service I give and in accordance with accepted professional behavior.
In couples therapy, I follow a no secrets policy when working with couples. This policy states that anything shared in I individual therapy can be brought up by the therapist at any time during a couples session.
Termination of therapy
The goal of therapy is to develop skills to help points reach their goals, once both therapist and client agree that goals have been met an aftercare plan will be developed and our sessions will conclude. You have the right to terminate therapy at any time.
Agreement
I have read this information fully and completely. By clicking below I fully acknowledge that it is my choice to participate in therapy. I understand that outcomes of therapy depend upon my personal investment in the process. I acknowledge responsibility for payment of these services. I understand the social media policy.
The informed consent will be covered during your first session, any questions you might have please ask. Any update to the informed consent will be shared during your session.
